Description

Przedmiotem zamówienia jest zakup i sukcesywna dostawa leków do programów lekowych. Leki muszą być zarejestrowane w programach lekowych B.29, B.133, B.147. Wykonawca oświadcza, że zaoferowane produkty lecznicze w zakresie przedmiotu zamówienia są zarejestrowane ze wskazaniem do stosowania w programie lekowym, zgodnie z aktualnym obwieszczeniem Ministra Zdrowia. Szczegółowe wymagania dotyczące realizacji zamówienia zostały określone we wzorze umowy oraz formularzu asortymentowo-cenowym stanowiącymi odpowiednio załącznik nr 5 do SWZ i załącznik nr 2 do SWZ.
