Description

The subject of the contract are successive deliveries of infusion fluids, medicinal products and foodstuffs for special nutritional purposes to the Hospital Pharmacy of the Health Care Team in Bolesławiec. The detailed description of the subject of the contract includes the Price Form - Annex No. 2.1-2.4 to the SWZ. Each Contractor may submit only one tender for the entire contract or the task(s) concerned in writing, failing which they shall be null and void. If the Contractor submits more than one tender for the entire contract or for a given task/tasks, all tenders of that Contractor will be rejected by the Contracting Authority. The order consists of 4 tasks. The contracting authority shall allow partial tenders to be submitted. As a partial offer, the Ordering Party will consider the offer submitted for all items in a given task. The contracting authority does not allow the submission of tenders for individual items. The Ordering Party stipulates that the offered subject of the contract must belong to the group of: medicinal products in accordance with Article 2 (2); 32 and Art. 40 of the Pharmaceutical Law Act of 6 September 2001 (Journal of Laws 2023, item 1938), (b) foodstuffs intended for particular nutritional uses in accordance with the Regulation of the Minister for Health of 16 September 2010 on foodstuffs intended for particular nutritional uses (Journal of Laws 2015, item 1026). The offered products must have appropriate certificates or attestations allowing their use in health care in accordance with Polish law and European Union law.
