PL-Leszno: medical equipments
The subject of the contract is the purchase of medical equipment for the development of modern diagnostics and treatments in the gynaecological and obstetric area for the Provincial Hospital Zespolone.
Supply of new medical devices with installation and start-up, including training of staff, broken down into 3 tasks – Packages:
Package 1.
1) Multi-stationary obstetric surveillance system – 2

USG with colour Doppler and 3D/4D imaging system – 1
Package 3.
1 operational hysteroscope – 2

Main subject matter of the contract by Common Procurement Vocabulary (CPV): 33100000-1.
A detailed description of the subject-matter of the contract, technical and performance data can be found in Annex 18.2.1-3 to the TSI.
The contractor shall deliver the subject matter of the contract at its own expense and in its own endeavour.
Installation and commissioning of the subject of the contract will take place at the place of use indicated by the Contracting Authority.
Performance of the contract means the supply of apparatus, the installation (installation) of the equipment supplied, the training of the Contracting Authority’s staff in the handling, operation and use of the subject of the contract, the provision of detailed instructions for use, maintenance of equipment and information materials with technical parameters in Polish.
The contracting authority requires the provision of a quality guarantee for the subject-matter of the contract for a minimum period of 24 months, and the provision of a guarantee service during the warranty period, without charging separate fees.
The procurer expects high-quality equipment, i.e. free from any defects, reliable, fulfilling absolutely all the functions for which it is intended, ensuring the safety of patients and the medical staff performing the tests using it.
The equipment forming the subject-matter of the contract must not affect the operation of other facilities, particularly for the provision of health services. The subject of the contract offered must comply with the legal requirements relating to the authorisation for medical marketing and use in the provision of health services, comply with all the standards and parameters laid down by Polish law and by European Union law in this respect, and hold all necessary attestations and registration certificates relating to the subject matter of the contract covered by this ToR, in accordance with the provisions of the Medical Devices Act of 20 May 2010 (Journal U. of 2010 No 107, item 679 late. as amended).
The equipment which is the subject of the contract must bear the CE marking.
Whenever the subject-matter of the contract is described in these ToR or in the annexes to the ToR by indicating its own names, these indications should be taken as an example of a product which corresponds precisely to the description of the subject-matter of the contract and the Contracting Authority allows equivalent solutions to be used, provided that the equivalent product proposed by the Contractor meets the minimum requirements of the same class as the Contracting Authority expects.
