Description

1. The subject of the contract is the supply of disposable equipment I - DOGRYWKA. The subject-matter of the contract was divided into 13 TASKS in accordance with Annex 1 to the Assortment and Price Form. A detailed description of the subject of the contract, including the required parameters, size, feature, IU, quantity is specified in the ASSORTYMENT AND PRICE FORM, constituting Annex No. 1 to the SWZ. The parameters described in Annex 1 to the SWZ, as required parameters, are boundary requirements, the non-fulfilment of which will result in the rejection of the tender. 3. The detailed conditions and rules for the performance of the subject of the contract are set out in the Model Contract – Annex 5 to the SWZ ‘Model Contract’. 4.Series and expiry date on unit packet and invoice. 5. The expiry date of the offered assortment, not shorter than 12 months, counting from the date of delivery to the Ordering Party. 6. The Contractor will deliver the subject of the contract to the APTEKI SZPITALA Warehouse. 7. The offered single-use equipment is a medical device and must be allowed to be placed on the market and used in accordance with the applicable regulations, i.e. in accordance with the conditions set out in the Act of 7 April 2022 on medical devices (Journal of Laws of 2024, item 1620) and its implementing acts, and must hold applicable CE Certificates, Declarations of Conformity and other documents required by law.
