The subject of the contract is the lease of biochemical-immunochemical platforms and an immunochemical analyzer together with the supply of reagents, calibrators, controls, rinsing fluids and consumables – the subject of the contract was divided into 2 lots, i.e.: ◦ Package 1 – Biochemical-immunochemical platforms ◦ Package 2 – Immunochemical Analyzer 2. A detailed description of the subject of the contract includes: ◦ Annex 2a – Package 1 (Assortment and Price Form) ◦ Annex 2b – Package 2 (Assortment and Price Form) ◦ Annex 3a – Compilation of Technical Parameters Package 1 ◦ Annex 3b – Compilation of Technical Parameters Package 2 3. The offered medical equipment should meet the requirements set out in the Act of 7 April 2022 on medical devices. 4. The biochemical-immunochemical platforms offered in Package 1 must be manufactured not earlier than in accordance with the provisions of Annex 3a to the FTZ – Summary of Technical Parameters Package 1, complete, free from design, material and implementation defects. 5. The immunochemical apparatus offered in package 3 must be manufactured no earlier than 2024, complete, free from defects in construction, materials and workmanship. 6. The offered subject of the contract must have the CE marking, be in accordance with the requirements of the Ordering Party described in the tender specifications and with the provisions of law in force in Poland, have the required declarations of conformity, attestations, certificates of registration, certificates of release for free circulation and use in Polish health care institutions on the basis of the applicable provisions of law. 7. The Contractor shall bear the cost of deliveries and insurance and shall be responsible for the delivery of the subject of the contract to the Nowodworski Medical Centre. 8. Delivery of reagents, calibrators, controls, rinsing fluids and wear parts will be carried out according to the needs of the Ordering Party in the quantities and frequency agreed upon by order, sent by e-mail. 9. The Contractor undertakes to supply reagents, calibrators, controls, rinsing fluids and consumable parts within a maximum of 5 days from the date of order submission. 10. The subject of the contract will be delivered to the laboratory of the Nowodworski Medical Center in Nowy Dwór Mazowiecki, ul. Honey 2 on working days in hours. 8.00-20.00. 11. The quantities given in Annex 2a/2b to the FTZ are only indicative and may be changed during the implementation of the contract in connection with the needs of the Ordering Party. The Ordering Party undertakes to perform not less than 50% of the contract value. 12. The Contractor undertakes to deliver and install individual devices within 30 calendar days from the date of conclusion of the contract to the laboratory of the Nowodworski Medical Center in Nowy Dwór Mazowiecki, ul. Honey 2 on working days. 13. The delivery of the leased platform includes: ◦ installation and installation at the Ordering Party ◦ start-up ◦ providing the Ordering Party with instructions for use in Polish ◦ staff training. 14. After installation and commissioning, the device must be fully ready for operation without incurring any additional costs by the Ordering Party 15. The Contractor shall carry out, at his own expense, the training of staff in the operation of the equipment supplied.
