Poland-Nowa Dęba: Medical equipments
1. Subject of contract:
1.1. The subject of the contract is ‘Supply of medical equipment and equipment for the Anaessiology and Intensive Therapies for the Self-Self-Self-Self-Public Healthcare Team in Nowa Dęba’.
1.2. The subject of the contract is carried out as part of a project entitled ‘Refurbishment of the Anesthesia and Intensive Therapies Branch to Building No 1 – Improving the quality and access to the medical services provided by SPZZOZ in Nowa Dęba’.
Project co-financed by the European Regional Development Fund under the Podkarpackie Regional Operational Programme 2014-2020.
1.3. A detailed description of the subject-matter of the contract, the type and minimum parameters are set out in Annex 2 to the ToR – Product and Price Form and in Annex 3 to the ToR – Description of the subject matter

1.4. The subject of the contract covers the delivery to the Contracting Authority’s premises, the commissioning and putting into service and the training of staff in the operation of the medical equipment and equipment offered.
1.5. The contracting authority divided the subject matter of the contract into 22 lots/packages and allows partial tenders to be submitted (Article 2(6) of Article 36(2)(1) of the Public Procurement Act).
1.6. The part for which a tender may be submitted should be understood as a package. Each lot/package is a separate subject of the contract.
1.7. Tenders may be submitted in one or all lots.
1.8. The contracting authority shall not allow the submission of tenders for individual items in packages.
1.9. The subject-matter of the contract offered by the Contractor must be pre-formulated, conforming to the specifications, must be complete and ready for work when installed (installed) without any additional purchases and investments.
1.10. Delivery of the subject matter of the contract to the premises of the Contracting Authority Ms Skłodowskiej 1A, 39-460 New Dęba provides the Contractor with the costs and risk of transport during the running hours of the Contracting Authority, i.e. hours. 7:00 to 14:35.
1.11. The products offered by the Contractors in this procedure must comply with the requirements laid down in the Medical Devices Act of 20 May 2010 (codification: Journal of Laws 2017, item 211) i.e. CE certificate or declaration of conformity (if required by law)
1.12. During the warranty period, the Contractor will carry out, free of charge, repair, maintenance and maintenance of medical equipment and equipment supplied and installed in accordance with the Medical Devices Act of 20.5.2010, with particular reference to Article 90 paragraph. 3-5 of that law.
