Description

1. The subject-matter of the contract is the purchase and successive supply of medicinal products used in drug and chemotherapy programmes listed in 13 lots for the Fryderyk Chopin University Clinical Hospital in Rzeszów 2. Detailed description of the subject of the contract, the Contracting Authority is specified in the Price and Tender Forms – Attachments No. 1 for individual parts, which are an integral part of the SWZ. 3. A detailed description of the subject-matter of the contract is set out in the Price and Tender Form – Annex 1, which is an integral part of the SWZ. 4. The Ordering Party requires to offer the subject of the contract, which: a) meets the requirements described in the Price and Offer Form, b) meets the requirements described in the CVT, c) is covered by the Act of 12 May 2011 on the refund of medicines, foodstuffs intended for particular nutritional uses and medical devices – applies to medicinal products covered by the refund. 4. The scope of the contract includes the delivery of the items of the contract in the quantities specified in the Price and Offer Forms for the period of: - Lot No 1 to No 7 – from 27 March 2025 for 12 months, with the possibility of early implementation if the quantities under the current contract are exhausted. – Lot No 8 to Lot No 12 – for 12 months from the date of conclusion of the contract – Lot No 13 – from the date of conclusion of the contract until 20 September 2025. 5. The Ordering Party in Sections 1 to 13 allows for the possibility of changing the term of the contract concluded with the Contractor in the event of failure to perform the entire subject of the contract during its term. In this case, the amendment of the contract requires the signing of an annex, except that the Contracting Authority shall unilaterally notify the Contractor of the proposed extension of the contract before the expiry of the period for which it was concluded.
