Przedmiotem zamówienia jest dostawa leków, gazików do dezynfekcji, podłoży transportowych oraz zrównoważonego roztworu soli fizjologicznej dla okulistyki z wykazem asortymentowo-cenowym stanowiącym zał. nr 2 do SIWZ.
Załącznik ten jest obowiązujący i nie można dokonywać w nim żadnych zmian.
Zamawiający dopuszcza składanie ofert na części:
— część nr 1 – dostawa leków,
— część nr 2 – dostawa leków,
— część nr 3 – dostawa leków,
— część nr 4 – dostawa leków,
— część nr 5 – dostawa leków,
