Description

1. The subject of the contract is the development of the design documentation for the investment task entitled “Reconstruction of the Neurosurgery Clinic in the Clinical Voivodeship Hospital No. 2 of St. John Paul II in Warsaw. The Queen’s Jadwiga in Rzeszów. The task includes the development of 2 (two) complete design documentation for: a) “Reconstruction and renovation of part of the rooms of the 2nd floor for the needs of the Cardiology Clinic in the Clinical Voivodeship Hospital No. 2 of St. John Paul II” Jadwiga Queen in Rzeszów, and (b) “Reconstruction and renovation of the premises of the 4th floor of Building A for the needs of the Neurosurgery Clinic in the Clinical Voivodeship Hospital No. 2 of St. John Paul II” Jadwiga Queen in Rzeszów" and c) performance of author’s supervision covering construction works carried out on the basis of prepared design documentation. The design documentation will be prepared on the basis of the requirements and comments of the Users provided by the Ordering Party, the description of the Order Subject and on the basis of administrative decisions. The scope of the task includes obtaining all necessary decisions, permits, arrangements, derogations and other materials and components resulting from legal provisions, the acquisition of which will be necessary for the notification and execution of construction works on the basis of the prepared design documentation. The scope of the study includes: a) Preparation of a complete design documentation for the reconstruction and renovation of part of the rooms of the second floor, located on the eastern side of Building A, with a total area of approximately 622 m². (b) Preparation of a complete design documentation for the reconstruction and renovation of the premises of the 4th floor of Building A, with a total area of approximately 1 283 m².
