According to the European SAMIRA Action Plan1, reinforced by the EESC and the Council positions, securing the supply of medical radioisotopes in the medium to long term to maintain EU patients’ access to the related medical procedures is vital. Related actions are brought together in ERVI (European Valley Radioisotope Valley Initiative), the feasibility and basic design of which are outlined in this study. These actions pertain to i) investments in the supply chain, ii) a comprehensive R&D&I program on medical isotopes and iii) a more reliable monitoring and forecasting of medical isotope use in the European Union. Diverse ERVI organisational and funding options are explored, and a preliminary impact assessment of ERVI is performed. The evolving initiatives of the new European Commission present a unique opportunity to define and secure ERVI's future. Instruments such as the Critical Medicines Act, the Competitiveness Fund, and the accelerated implementation of IPCEIs have the potential to significantly strengthen ERVI’s objectives, reinforcing Europe’s autonomy, competitiveness, and leadership in medical isotopes and nuclear medicine. As a first step in a deliberate, stepwise approach, the ERVI-Standing Coordination Committee builds on the current ERVI Steering Group while expanding participation to the full spectrum of relevant stakeholders
