Gegenstand des Vertrages ist eine Gruppenversicherung für das Leben von Angestellten, Ehepartnern und erwachsenen Kindern von Angestellten der Stadt Rybnik. Der Versicherungsumfang umfasst: 1. Versicherung gegen Tod des Versicherten und zusätzlich: 1) insurance against the death of the Insured following an accident, 2) insurance against the death of the Insured following an accident at work, 3) insurance against the death of the Insured following a motor accident, 4) insurance against the death of the Insured following a motor accident at work, 5) insurance against the death of the Insured following a heart attack or stroke, 2. insurance against the death of a spouse and, in addition, insurance against the death of a spouse following an accident, 3. insurance against the death of parents or in-laws, 4. insurance against the death of a child, 5. insurance against the birth of a child, 6. insurance against the birth of a dead child, 7. insurance against the orphanage of a child, 8. insurance against permanent damage to the health of the Insured following an accident, 9. insurance against permanent damage to the health of the Insured following a heart attack or stroke, 10. insurance against serious illness of the Insured, 11. insurance in the event of surgical operations of the Insured, 12. insurance in the event of specialist treatment of the Insured, 13. insurance of the Insured's treatment in the hospital in connection with illness (including stay in the ICU and convalescence) and additionally insurance of the Insured's treatment in the hospital caused by heart attack or stroke, 14. insurance of the Insured's treatment in the hospital in connection with personal injuries suffered as a result of an accident (including stay in the ICU and convalescence) and additionally: 1) Versicherung der Behandlung des Versicherten im Krankenhaus im Zusammenhang mit Personenschäden infolge eines Arbeitsunfalls, 2) Versicherung der Behandlung des Versicherten im Krankenhaus im Zusammenhang mit Personenschäden infolge eines Verkehrsunfalls, 3) Versicherung der Behandlung des Versicherten im Krankenhaus im Zusammenhang mit Personenschäden infolge eines Verkehrsunfalls bei der Arbeit. Die Zahl der Beschäftigten im Rathaus betrug zum 6. November dieses Jahres 472 Personen, der Besteller behält sich jedoch das Recht vor, die Zahl der Versicherten unter Beibehaltung der im Angebot angegebenen Prämie zu ändern.
